After the maintenance personnel completed the procedures such as operation plate registration, Lockout and tagout, they welded a steel plate onto the material receiving device. After the welding was completed, the maintenance personnel left the site and resumed production at 14:20 after completing the plate return and power supply procedures. Subsequently, during the inspection operation, the inspector failed to follow the requirements of the "Management System for Operation and Maintenance Plates of the Bar Plant of a certain steel Company" to handle the procedures for hanging the operation plate and shutting down or cutting off the power. Instead, he risked standing between the running blanking roller conveyor and the flush baffle to measure and was hit by the flush baffle by the round steel that automatically returned to the flush position on the blanking roller conveyor, resulting in his death.
