Process Empowerment: Making the Lockout Tagout Culture a Well-Defined Process
The
cultivation of culture cannot be achieved without the support of
standardized processes. Our city takes the "Seven-step Standard
Procedure" as the core and integrates the lockout tagout culture into the entire operation process, transforming the culture from "soft power" to "hard guarantee".
In
terms of process design, we strictly follow the closed loop of "energy
identification - shutdown and power disconnection - physical isolation - lockout tagout - energy release - verification and
confirmation - on-site supervision". Each step has clear operation norms
and responsible entities. To enable workers to clearly understand the
process, our city has produced visual flowcharts, templates, and posted
them at prominent locations in the operation site; an online simulation
operation platform has been developed, allowing workers to conduct
virtual drills at any time and familiarize themselves with the key
points of each operation step. At the same time, we uniformly provide
"six-one" standard toolkits to maintenance personnel, including
customized locks, voltage detectors, grounding wires, etc., to ensure
the implementation of the process through hardware support.
For
special scenarios such as confined spaces, high-voltage electrical, and
chemical pipelines, 12 special supplementary procedures have been
formulated to ensure that the cultural construction is precisely adapted
to actual operations. A coal mining enterprise, in line with the
characteristics of underground operations, deeply integrated the "lock
culture" with team management, implemented the mechanism of "lock number
- responsibility to individuals - process traceability", ensuring that
each lock has an "owner" and each step has "supervision", completely
solving the problem of unclear responsibilities in the past.
Mechanism Guarantee: Ensuring the Long-Term and Sustainable Implementation of Cultural Construction
To ensure the implementation and long-term operation of the lockout tagout culture, our city has established a "culture cultivation + supervision
and assessment + responsibility traceability" full-chain guarantee
mechanism.
In
terms of supervision and assessment, we adopt methods such as "four
nots and two direct" inspections, cross-checks, and special supervision,
going deep into the enterprise's production workshops to focus on
verifying the execution of the lockout tagourt process,
the standardization of lock usage, and the authenticity of the
information. The effectiveness of cultural construction is included in
the enterprise's safety production credit rating. For enterprises that
actively practice the lockout tagout culture and
achieve zero accidents during maintenance, they will be commended and
rewarded, and their experiences will be promoted; for enterprises with
lagging cultural construction and frequent violations, a rectification
notice will be issued and a re-examination will be conducted within a
deadline. Those who refuse to rectify will be punished strictly
according to law.
In
terms of responsibility traceability, a "three-tier system" of
"operation ticket + lock code + electronic filing" is implemented. Every
maintenance operation must fill out an electronic operation ticket, the
lock code is bound to the operator's information, and the entire
operation process is recorded and filed through video surveillance or
on-site supervision. This ensures that "whoever operates is responsible,
whoever violates is held accountable". At the same time, we have opened
up channels for public supervision, set up hotlines and online
reporting platforms, and encourage workers and the public to supervise
violations of operating procedures, forming a "full participation, full
supervision" favorable situation.
Data Witnessing Results: Since the lockout tagout culture was fully promoted, the number of equipment maintenance
accidents in key enterprises in the city has decreased by 100%,
achieving the goal of zero accidents during maintenance; the compliance
rate of operations has increased from 65% before implementation to 98%,
and more and more enterprises have built a solid safety production
defense line through solid cultural construction. A steel enterprise
that previously experienced mechanical injury accidents due to omitting
the verification step has achieved 24 consecutive months without similar
hazards after implementing the lockout tagout culture,
through the infiltration of concepts, standardization of processes, and
constraints of mechanisms. Establish an enterprise experience exchange
platform, promote innovative practices such as "program visualization
management" and "intelligent lock traceability", and enable the "lockout
tagout" culture to take root in more industries, thereby laying a solid
cultural foundation for high-quality development in terms of safety
production.
